Appointments can be canceled or rescheduled up to 24 hours before the original appointment at no charge. Please provide at least 24 hours’ notice if you need to cancel or reschedule. Any cancellations or rescheduling made less than 24 hours in advance will incur a cancellation fee of 50% of the service cost. No-shows will also be charged the cancellation fee.

Draping will be used during sessions, with only the area being worked on uncovered. Clients under 18 must have a parent or legal guardian present to authorize the therapeutic massage session and must be with a same-gender therapist. Clients under 17 must have a parent or legal guardian present to authorize a facial session.

It is my choice to receive spa treatments. I understand the treatment is for the well-being of my body and mind, and I agree to communicate with my service provider if I feel my well-being is compromised. I understand that service providers do not diagnose illness, disease, or any physical or mental disorder, nor prescribe medical treatment or pharmaceuticals.

Spa services are not a substitute for medical examination or diagnosis. It is recommended that I consult a primary health care provider for medical services. I have disclosed all known medical conditions and will update the service provider with any changes in my health status.

I understand and voluntarily accept the risks associated with services such as massage, cellulite-reduction, lymphatic drainage, facials, and the use of equipment. I acknowledge and voluntarily assume the risk of injury, accident, or death that may arise from any of the services provided. Ama.s Body Care will not be liable for personal, bodily, or mental injury, economic loss, or damage resulting from negligence or other acts by Ama.s Body Care or its representatives, to the fullest extent permitted by law.

This waiver applies to all services, including but not limited to: Massage, Facials, Cellulite-Reduction, Lymphatic Drainage Massage, Blanket Sauna, or use of any equipment. I agree that this waiver is effective for all future services until specifically requested otherwise. Any inappropriate behavior during services will result in termination, and full payment will be expected.

By signing this form, I agree to these terms and release Ama.s Body Care and its employees from any liability.
